Czech National Marrow Donors Registry (ION-5440)

CNMDR has adopted following preliminary policy for donors after vaccination against COVID:

  • mRNA vaccines (Pfizer / BioNTech, Moderna) and protein vaccines (Novavax, Sanofi) – deferral of 7 days after each vaccination dose, provided no relevant side effects occurs.
  • Vector vaccines (Astra Zeneca, Johnson & Johnson) – deferral of of 28 days after each vaccination dose (a change of opinion after gaining more experience with vaccination cannot be ruled out).
  • Information about the donor's vaccination sent to the transplant center

With increasing experience the policy is likely to be modified.

Germany ZKRD (ION-6939)

Germany does currently not have any specific regulation as far as SARS-CoV-2 vaccination and stem cell donation is concerned. The decision if a donor is eligible for PBSC donation after (any) vaccination is generally made by the collection center's physician. They will consider European recommendations (https://www.ecdc.europa.eu/en/publications-data/coronavirus-disease-2019-covid-19-and-supply-substances-human-origin(next to the German national guidelines), but decisions are made individually.

Italian Bone Marrow Registry (7450)

IBMDR according to the Italian national requirements, applies this policy for  COVID-19 Vaccination for Donors:

Deferral period after COVID-19 vaccination

  • Donors (selected for TC/Work up and donation) and  vaccinated with a vaccin based on weakened/ attenuated form of Sars -COV 2  -> deferral period of ( at least) 4 weeks after each vaccin injection
  • Donors selected for PBSC or Bone Marrow donation and vaccinated with a vaccin based on mRNA or protein subunits, with no alive virus  - > deferral period of (at least) 21 days after injection of the vaccin. This timing refers to the interval between vaccination and start of G-CSF or general anaesthetic for BM donation.
  • Donors selected for lymphocyte donation (DLI) and vaccinated with a vaccin based on mRNA or protein subunits, with no alive virus  - > deferral period of 3 -7 days (3 days minimum, 7 days recommended) after injection of the vaccin
COVID-19 vaccination after HPC or MNC donation:
  • It is recommended to document a fully recovery of the donor's blood values after the collection before  planning the vaccination.

Singapore Bone Marrow Donor Programme (ION-3785)

BMDP guideline for COVID-19 Vaccination for Donors

Any donor who wants or is required to be vaccinated for COVID-19 should receive the vaccine when it is available to them.

For both donor and patient safety, BMDP sets the following guideline for donor receiving COVID-19 vaccination:

  • For BM and DLI collection: Donor should receive the vaccination at least 14 days before the planned collection date.
  • For PBSC collection: Donors should receive the vaccination at least 14 days before the planned start of GCSF.

If donor develops symptoms after vaccination, BM donation/Start of GCSF will be deferred for 7 days after complete remission of symptoms.

For both PBSC and BM donors, vaccination should be delayed at least 3 days after donation to allow for post-donation recovery.

Switzerland Swiss Registry for Blood Stem Cell Donors (ION-9341)

Deferral period after COVID-19 vaccination:

  • For Swiss donors, the deferral period after COVID-19 vaccination is 7-14 days (7 days minimum, 14 days recommended). This timing refers to the interval between vaccination and start of G-CSF or general anaesthetic for BM donation. In Switzerland, only mRNA based vaccines are administered.

COVID-19 vaccination after HPC or MNC donation:

  • A delay of at least 7 days is recommended after the collection until a planned vaccination.

United Kingdom

United States of America NMDP-Be the Match (ION-3553)

While we are providing guidelines for the timing of vaccination as it relates to collection and filgrastim administration, any donor who wants or is required to be vaccinated should receive the vaccine when it is available to them.

  • For collection: If possible, the donor should receive the vaccination more than 72 hours before the proposed collection date.
  • For filgrastim administration: If possible, the donor should receive the vaccination more than 72 hours before the proposed start of filgrastim to distinguish between possible side effects of the vaccine and filgrastim itself (aches and pains, headaches, etc.).
