General instructions

• If you have any problems or questions regarding the GTR questionnaire, please email monique.joris@wmda.info

• Your GTR questionnaire will be sent to you by the WMDA CRM database providing a secure link to an excel file. WMDA will only provide access to registered GTR contact persons in your organisation. Please let us know if you did not receive the link or if we need to change the contact person(s). Please also do not forget to check your spamfolder.
• There are 3 sets of questions: those for donor registries, cord blood banks and donor registries with affiliated cord blood banks. Please check your questionnaire if it contains all relevant questions. Questions not relevant for your organisation will be hidden and you cannot unhide them yourself. If there are questions missing or there are too many questions, please let us know and we will correct your organisation type.

• Please fill the excel file online (in the sharepoint space). It is possible to download a version, but you will not be able to upload it. We will only accept questionnaires which are filled online.
• The WMDA GTR project manager can access your questionnaires online and provide assistance in case anything is unclear.

• The GTR questionnaire contains calculated fields which you do not have to fill. These fields have a light green answer field.
• The GTR questionnaire contains validation checks. If the validation does not hold true with the data you have filled an error will appear in the answer field. You will have to check the error and correct your data accordingly. WMDA will not accept questionnaires with errors.
• Each question has a unique code (WGQN number), which will not change in the future. Please refer to these numbers when defining your queries, when you have a question for us or when checking the calculated and validation fields.

• The GTR questionnaire provides guidance for most questions in the questionnaire itself. For further clarifications, please see the list of definition on the right part of this page.
