Please let us know if you did not receive the link or if we need to change the contact person(s). Please also do not forget to check your

• spam folder.
• There are 3 sets of questions: those for donor registries, cord blood banks and donor registries with affiliated cord blood

• bank(s). Please check your questionnaire if it contains all relevant questions. Questions not relevant for your organisation will be hidden and you cannot unhide them yourself. If there are questions missing or there are too many questions, please let us know and we will correct your organisation scope.

Organisation scopes

Scopes define what type of questions are included for a specific type of organisation.

• If your organisation is a Cord Blood Bank, the scopes CBB and DRCBB apply to you
• If your organisation is a Donor Registry, the scopes DR and DRCBB apply to you
• If your organisation is a Donor Registry with affiliated Cord Blood Bank(s), the scopes DR, CBB and DRCBB apply to you

How to fill your questionnaire

• Please fill the excel file

• in your online excel app. It is possible to download a version, but you will not be able to upload it. We will only accept questionnaires which are filled online.
• The WMDA GTR project manager can access your questionnaires online and provide assistance in case anything is unclear.
• Only fill out 0 if the answer is 0
• If the answer is unknown, fill out UNK or choose the unknown option if there is one
• If the answer is not applicable, fill out NA
• Do not leave any answer fields empty (except for the country list)

Specifications of the questionnaire

• The GTR questionnaire contains calculated fields which you do not have to fill. These fields have a light green answer field.
• The GTR section headers (in blue) and question sub headers (in orange) also do not require an answer.
• Each question has a unique code (WGQN number), which will not change in the future. Please refer to these numbers when defining your queries, when you have a question for us or when checking the calculated and validation fields.
• The GTR questionnaire contains validation checks. If the validation does not hold true with the data you have filled an error will appear

• next to the answer field. You will have to check the error and correct your data accordingly. WMDA will not accept questionnaires with errors.

• In WGQN-1500 (Check errors field) on the top of the form you can enable validation messages by setting this value to anything else than "no".
• In the second tab of your questionnaire file all validations are also displayed.

• When you have finished filling the GTR questionnaire you can fill out 'finished' in WGQN-1520 (Reporting status) at the top of the form. We are not automatically notified of this, so it would be great if you could send a message to monique.joris@wmda.info.
